Instructions for Using the Wayne After-Image Flash

The Wayne After-Image Flash is an electronic instrument designed for producing a long lasting after-image on the macula to test and develop central fixation. It is an easy to operate unit which provides immediate biofeedback information for both the patient and the therapist.

The Wayne After-Image Flash was designed for use in an optometric visual therapy program for remediating the following visual conditions: eccentric fixation; intermittent exotrope; anomalous correspondence; and for developing rapid and accurate saccadic and pursuit skills while maintaining accurate performance of each eye as well as bi-ocularity and binocularity.

An intense image is flashed onto the macula area of the retina. This sustained image provides a biofeedback indicator to the patient of each eye's performance. By flashing a vertical pattern onto the macula of one eye and a horizontal image onto the other eye the patient gets feedback which will enable a correction for proper correspondence. Flashing a (+) pattern onto the macula of a non-suppressing eye (properly fixating eye), patching this eye, and having the patient fixate on an external target patterns will permit the patient to visually "see" the amount of eccentric fixation to be corrected.

Operating Instructions

SLIDE the white button switch to "ON." A red dot should be visible under the switch. After six seconds a pulsating red light becomes visible under the red dot (if the pulsating light is not visible because either the unit has been in constant use or has not been used for a long time, then it will be necessary to charge the battery. To do so, merely plug in the charger overnight).

The After Image (AI) can place a vertical line pattern, or a (+) plus pattern on the macula. Hold the unit in an upright position so that the pattern faces the patient. For a vertical line pattern hold the case vertically. For a (+) pattern, flash a vertical pattern on the macula, and then rotate the case to a horizontal position (wait 4 seconds ) and flash a horizontal pattern on the same eye.

Activate the flash by depressing the small flash button.

It is suggested that the patient be either sitting or standing in a comfortable balanced position. The flash unit should be held approximately 16 inches from the eyes. Patient is instructed to fixate precisely at the red target dot in the center of the (+) Therapist must make sure the fixation is correct.

Patient should be instructed how to maintain and enhance the after-image by blinking slowly.

An excellent method for increasing enhancement is to use the Wayne After-Image Enhancer. Use the Wayne After- Image Enhancer as follows: With one hand, hold the enhancer in front of both eyes. The thumb should be placed on the side facing the eyes, and the other fingers on the opposite side of the plastic. Wave the enhancer slowly up and down in front of the user's eyes. Patient continues to fixate through the Enhancer at the target patterns. The Wayne After-Image Enhancer will help the patient to maintain a strong after image.

To develop accurate fixation and tracking skills

Set the Wayne After-Image flash to the X pattern position.Instruct patient to accurately fixate on the red dot target on the after image flash. Check to be sure that both eyes are fixating correctly. After flashing the image, have patient face a white or silver screen. Patient should see a clear after image of the X. Instruct the patient to place the after image on various targets such as: Small fixation targets placed on the wall or ceiling. Targets on a Rotator disk such as the Wayne Robot Rotator.(use a program with variable speeds and directions). Targets on a swinging Marsden ball. Cover the front of a strobe light with black paper that has a 5mm center hole. Trace a pattern while the after image is centered on the tip of the pencil or crayon. Track a rotating projected spot of light using the Wayne Tachistoscope Rotator.

Training of an eccentric fixator (Transferred after image)

Patient should stand in a comfortable position maintaining good balance and posture. Occlude the eccentrically fixating eye. Hold the Wayne After-Image Flash unit approximately 16 inches in front of patient Instruct patient to fixate on the small red dot in the center of the cross.

Immediately after flashing a vertical, and horizontal image to form a (+) on the macula, place the occluder over the non-eccentric eye, and instruct patient to look at a white or silver wall or screen, and to transfer the image to the eccentric fixating eye. This may take a little time. When the patient has mastered this technique have patient attempt to fixate with the eccentric fixating eye.

Patient should be made aware that the after image position does not correspond to his fixation point. This is due to his eccentric fixation. The training consists of having the patient attempt to place the after-image over various static and dynamic fixation targets, and to maintain the image on the target.

It may be necessary to start the above procedure having the patient fixate off center when flashing the image onto the non-eccentric fixating eye. This can be done by painting a white dot onto a clear plastic tape, and placing it off center.

Patient can also place the after image onto a Hadiger Brush image. Use the Wayne After-Image Enhancer to maintain a clear after image.

Anomalous correspondance

Flash a vertical on one eye and a horizontal on the other.

Patient should stand in a comfortable position maintaining good balance and posture. Occlude the right eye. Hold the Wayne After-Image Flash unit approximately 16" in front of patient's left eye. Instruct patient to fixate on the small red dot in the center of the cross. Immediately after flashing, move the occluder to the left eye, and rotate the flash unit 90 degrees . Again have patient fixate on the red dot. Flash unit. Patient should now fixate on a white wall or silver screen, and a vertical /horizontal line after image should now be visible. Correct correspondence will result in a symmetrically formed X. Have patient blink, or use the Wayne After-Image Enhancer to maintain a clear after- image. Patient should now train with static and dynamic fixation targets.

Intermittent exotropes

Use the above procedure, and have the patient fixate and place the after image X onto the various target patterns. Instruct the patient if the X disappears to relax, blink and repeat the fixation until the X appears again.
