Orders are accepted by mail, phone, Fax, e-mail, or the online order form below. For security reasons, do NOT include credit card numbers in orders placed by e-mail or the order form. Be sure to include your phone and/or Fax number and your complete address. Upon receipt of your order, we will contact you for confirmation.

All prices are Net 30 Day for established accounts. New accounts must be pre-paid by check or credit card (Visa/Mastercard/AMEX). Domestic orders for the Wayne Saccadic Fixator require a $100 deposit. Domestic shipments are via UPS, FedEx or U.S. Postal Service. Freight charges will be added to the bill. Illinois residents add 9.0% sales tax.

A pro-forma invoice will be mailed or faxed for orders outside the continental USA. International orders must be pre-paid by wire transfer, bank check or credit card (Visa/Mastercard/AMEX). Add a 5% surcharge for export. International shipments can be by your preferred carrier or you can let us select the carrier. Whenever possible, freight and insurance is collect.

Merchandise received in damaged condition must be reported to us immediately for followup. No merchandise will be accepted for return without prior authorization.
